Конечно, стандартная форма (они же должны, как то узнать, что Вы им отправили и за что Вам возврат делать)....и чек об отправке.Или вы про стандартную форму возврата?

I'm very sorry to hear that we’ve sent you a faulty product.
I've issued an immediate replacement for you which should be with you by 21/09/12. It will be sent to the original delivery address (we're not able to replace it to any other address).
You’ll soon be receiving a despatch confirmation email from us which will provide the delivery details for you.
We kindly request that you return the faulty item back to us. Please use the sticky address label from the front of your returns note when you send the item back to us. If you haven't got this anymore, you can print a new one off by clicking here.
When you have sent the item back, please complete the attached form and fax (or scan and email) it, along with the postage receipt to +44 1442 840 278. We will then refund your returned postage costs.
Just fill in the section on the returns note to say the item is faulty, and then put the note in your parcel when you send it back.
It’s also important that you get proof of posting from your Post Office when you send it back, so if it goes missing in the mail, you can prove you sent it. Please send the parcel back within 28 days.
